Job Description: Case Manager
Family and Children’s Service has an opening for a full time Case Manager/Home Visitor for its Young Parent Support Program. Young Parent Support (YPS) is a home visiting/case management program that works with parents up to 23 years old in the communities of Chelsea and Revere. Under the supervision of the Program Director, this position will ensure the creation and implementation of quality transformational relationships with an average of 15 pregnant and parenting multicultural youth and the provision of excellent care and service through the delivery of home-based case management services (including assisted referrals, accompaniment, and interpreter support) as well as life skills training and competency-building programming. The case manager will also complete timely evaluations, assessments, and referral forms in order to ensure accurate delivery of services, and implement developmental screens (ASQ and ASQ/SE) with children involved in the program
Qualifications: Candidates will have a Bachelor’s or Associate’s degree, preferably in health science, behavior science, or general education courses in liberal arts, sciences, and the humanities, in areas such as addiction or child protection, with a minimum of 2-5 five years’ experience providing services to infants, children and families. Will also have ability to demonstrate extensive knowledge of community resources verifiable through reference check. Ideal candidates will have an ability to establish trusting relationships, and to think and act intentionally and strategically to help participants. Will also have capacity for creative problem-solving, conflict resolution, ability to work as part of a team, with strong verbal and written communication skills. Candidate must be able to travel with appropriately insured personal vehicle and maintain a valid MA Driver’s License; be computer literate and detail-oriented; with experience working with diverse cultures. Candidate will feel passionate and committed to directly working with young parents and their children. Must be Bilingual (English/Spanish, strongly preferred; other languages considered).
