Youth Support Specialist Project Advance

OVERALL POSITION:  The Youth Support Specialist will divide their time between two youth-focused programs: Project Advance (that provides case management, mentoring & advocacy to high-risk court- or gang-involved youth); and Project PRIME (a sexual health education program promoting the prevention of STD/HIV and unplanned pregnancies).  Both programs utilize a Positive Youth Development framework and follow trauma-informed and culturally competent practices to successfully engage ethnically diverse youth. 

PROJECT ADVANCE Duties & Responsibilities

  • Conduct an assessment resulting in an individualized service/action plan regarding the academic, vocational, emotional, social, financial and recreational needs of the participants
  • Provide direct case management, coaching and advocacy services on behalf of the clients
  • Coordinate and refer program participants to local social supports, family supports, educational supports, and after-school and summer activities
  • Have weekly face-to-face contact with each youth
  • Conduct home visits with the youth twice a month
  • Maintain good communications with collateral service providers and family members
  • Facilitate group activities for the youth involved in the program
  • Plan and facilitate recreational activities for all program participants
  • Participate in targeted outreach in coordination with the program outreach worker
  • Reach out to appropriate referral sources to secure referrals and provide consistent follow-up
  • Complete all required documentation, including youth and family assessments, service plan and updates, service delivery logs & contact notes, and other required documentation as determined by the program manager
  • Collect data and document program activities as needed for the program evaluation
  • Maintain client records in compliance with funding program and agency regulations

PROJECT PRIME Duties & Responsibilities

  • Organize and facilitate educational groups faithfully implementing the program’s approved curriculum
  • Show initiative in working creatively with program participants to engage youth and help them learn about and implement responsible pregnancy and STI/HIV prevention behaviors
  • Complete Community Health Worker and Sexual Health Education training, as needed

Common Duties & Responsibilities across Programs

  • Participate in activities and efforts to promote the programs (e.g.: participation in coalition and community meetings)
  • Educate staff/providers about target population and community characteristics and needs
  • Collect data and document program activities as needed for the program evaluation
  • Collaborate to enhance coordination, quality and efficiency in program services at F&CS and with other programs
  • Follow protocols as outlined by the project proposals and/or contracts
  • Maintain good communications with collateral service providers
  • Participate in staff development trainings and external educational offerings to maintain and improve knowledge
  • Participate in weekly supervision
  • Attend staff meetings as required
  • Maintain flexible hours to meet the needs of the program

Qualifications:

Ideal candidates will possess a Bachelor’s degree in Human Services or related field with 2-3 years of experience working with youth, particularly high-risk, ethnically diverse youth within urban settings.  Knowledge of relevant social support agencies and programs.  Experience providing educational workshops, particularly in the public health field is most desirable.  Knowledge of and experience with the juvenile or adult justice system is also a plus.  In place of degree, the candidate will have 3-5 years’ experience or greater working with the target population.  In addition to English, the candidate will effectively communicate in Spanish, and have a valid driver’s license with use of a vehicle for work purposes.
