Young Parent Support Program – Case Manager/Home Visitor

Family and Children’s Service has an opening for a full time Case Manager/Home Visitor for its Young Parent Support Program. Young Parent Support (YPS) is a home visiting/case management program that works with parents up to 23 years old in the communities of Chelsea and Revere. Under the supervision of the Program Director, this position will ensure the creation and implementation of quality transformational relationships with young parents and participate in the creation and delivery of case management, engagement, life skills, pre-vocational, and competency-building programming. The Case Manager/Home Visitor will demonstrate excellent care and service through the following responsibilities:

  • Engage in family recruitment and outreach activities
  • Provide home based case management services, including assisted referrals, accompaniment, and life skills training, to a caseload of up to 15 pregnant or parenting multicultural youth.
  • Create and maintain participant files.
  • Complete all required documentation including case notes and assessments.
  • Complete ASQ and ASQ/SE developmental screenings for all children involved in the program.
  • Make appropriate referrals to area resources based on family specific needs.
  • Provide ongoing outreach and follow-up with participants and communicate with collateral contacts as necessary and appropriate.
  • Attend monthly F&CS staff meetings, weekly individual supervision, training and other meetings/groups as required.
  • Plan and participate in participant focused group activities including classes, workshops, and special events as assigned.
  • Assist with emergencies and crisis intervention.
  • Other duties as assigned.

Qualifications: Applicants should have a Bachelor’s Degree in a human service field, experience working with families and children in an ethnically, culturally, and racially diverse environment, and be bilingual Spanish/English.  Ideal candidates will have an ability to establish trusting relationships, and to think and act intentionally and strategically to help participants reach goals. Computer literacy, and strong verbal and written communication skills are required. Applicants must have a valid MA Driver’s License and use of a dependable insured vehicle. Work hours are generally Monday thru Friday, 9-5, but applicants must have some flexibility in their schedule to meet the needs of the program.
