Parents as Teacher Home Visitor

Position: Parents as Teachers Home Visitor

Family and Children’s Service has an opening for a full time Parents as Teachers (PAT) Home Visitor.  The Parents as Teachers program promotes the optimal early development, learning and health of young children by supporting their parents and caregivers utilizing a home visiting model.  Under the supervision of the PAT Program Director, home visitors provide directed support to participants in three areas: parent-child interaction, development-centered parenting, and family well-being. 


  • Engage in family recruitment and outreach activities
  • Provide home based services to a caseload of 10-14 families
  • Complete an initial and annual family-centered assessment with each family
  • Develop, monitor, and review goals with each family
  • Complete initial and annual Developmental Screening using the ASQ ASQ/SE  and Health Review for each enrolled child
  • Make appropriate referrals to area resources based on family specific needs 
  • Support parents and children transition to other services as needed, including preschool, or  kindergarten
  • Complete and submit all required program documentation within PAT timeline. 
  • Facilitate and co-facilitate groups that promote social connections and positive parent child interaction
  • Attend monthly F&CS staff meetings, weekly individual supervision, trainings, and other meetings/groups as required
  • Other duties as assigned

Qualifications: Applicants should have a Bachelor’s Degree in a human service field, experience working with families and children in an ethnically, culturally and racially diverse environment, and be bilingual Spanish/English.  Applicants must have use of a dependable insured vehicle. Work hours are generally Monday thru Friday, 9-5, but applicants must have some flexibility in their schedule to meet the needs of the program.
