Community Health Worker

Program Description: The Massachusetts Pregnant & Parenting Teen Initiative seeks to improve the health outcomes of young expectant and parenting mothers, as well as enhance the educational, social, and economic outcomes that shape their health. The program supports expectant and parenting teens, women, and fathers to continue their education regardless of their life situation and the real and perceived challenges of early childbearing and parenthood; empower them with relevant knowledge and skills to be productive adults; provide and link them (and their families) to services to ensure they are healthy and make informed decisions about their health, including sexual and reproductive health; and support them to make successful and healthy transitions into adulthood and the labor market.

Responsibilities of the Community Health Worker:

Engage and provide case management services to 15 pregnant or parenting multicultural youth

Complete family assessments, create life plans, and provide life skills training

Conduct home visits, provide client accompaniment and interpreter support

Provide information, assisted referrals and advocacy as needed to secure concrete supports

Support the parent’s educational and career goals

Secure academic supports, and monitor and encourage school retention

Maintain weekly, bi-weekly, and monthly service levels as required by program standards

Conduct outreach, engagement, and re-engagement activities, as needed

Collect data and document program activities for the program evaluation

Create and maintain participant files manually and electronically according to F&CS and DPH standards

Maintain and implement all appropriate evaluation tools as instructed including ASQ and ASQ/SE

Participate in staff development trainings to maintain and improve knowledge base including the completion of 28-42 hours of State-recommended community health worker training

Develop and implement individual and group programming for young parents

Facilitate classes, activities, workshops, and special events as assigned

Participate in and attend community meetings and events

Work with other staff to ensure that participants access healthcare and birth control information

Provide ongoing outreach and maintain good communication with collateral service providers

Collaborate with other providers, F&CS staff, and community resources to enhance coordination, quality and efficiency in program services

Follow protocols as outlined by the project proposal and/or contract

Participate in weekly supervision

Attend staff meetings as required

Maintain flexible hours to meet the needs of the program

Other duties, as assigned

Qualifications: Candidates will have a Bachelor’s or Associate’s degree, preferably in health science, behavior science, or general education courses in liberal arts, sciences, and the humanities, in areas such as addiction or child protection, with a minimum of 2-5 five years’ experience providing services to infants, children and families. Will also have ability to demonstrate extensive knowledge of community resources verifiable through reference check. Ideal candidates will have an ability to establish trusting relationships, and to think and act intentionally and strategically to help participants. Will also have capacity for creative problem-solving, conflict resolution, ability to work as part of a team, with strong verbal and written communication skills.  Candidate must be able to travel with appropriately insured personal vehicle and maintain a valid MA Driver’s License; be computer literate and detail-oriented; with experience working with diverse cultures. Candidate will feel passionate and committed to directly working with young parents and their children. Must be Bilingual (English/Spanish, strongly preferred).
