As legally appointed fiduciary, the agency will provide a comprehensive assessment of the elder’s psychosocial, medical, fiscal, and legal needs and implement an appropriate plan to meet those needs. This may include fiscal management of income and bills, applications for entitlement programs such as SSI or Medicaid, sale of home, nursing home placement, and/or general case management. Each client is serviced by a licensed social worker and a Family & Children’s Service’s retained attorney.
Referral Source: Families, attorneys, physicians, hospitals, and social service agencies. Acceptance of the client is made on a case-by-case basis.
